One Night Only Clinic November 29, 2015:

7th Grade and 8th Grade Sign Up

$20/clinic/player

 

1. Fill out the registration form below: 

         -if you have any trouble, please email standardhighvolleyball@gmail.com

 

2. Submit payment:

        -Mail check for $20 to "SHV" at 2209 Eagle Ridge Rd, Champaign IL 61822 OR pay by cash by bringing it to the first session

 

3. Register with the USAV:

       - All players must be registered with USAV. 

       - We will need a copy of the membership card and medical form at the first practice.
