How to sign up

Grades 5th-8th

1. Fill out the registration form below: 

         - Complete the form below
 

2.Register with the USAV (for insurance):       

         - Sign up for the Volleykids membership for $12/player for 10 weeks

         - All players must be registered with USAV.        
         - We will need a copy of the membership card and medical form at the first practice. The website for usav can be tricky the first time, don't get discouraged! Just email standardhighvolleyball@gmail.com if you need help. 

 

3. Submit payment:       

         - Mail your check to 3620 Freedom Blvd., Champaign IL 61822
