Back 2 School: 4-6th and 7-8th Sign Up

$200/10 sessions
or $140/5 sessions

 

 

1. Fill out the registration form below:

2. Submit payment:

        -Mail check for $140 to "SHV" at 2209 Eagle Ridge Rd, Champaign IL 61822 OR pay by cash by bringing it to the first session

 

3. Register with the USAV:

       - All players must be registered with USAV. 

       - We will need a copy of the membership card and medical form at the first practice.
